A cheap regimen of vitamins in use for decades is seen by scientists as a way to delay the start of Alzheimer’s disease and dementia, a goal that prescription drugs have failed to achieve.

Drugmakers including Bristol-Myers Squibb Co., Pfizer Inc. (PFE) and Eli Lilly & Co. (LLY) have spent billions of dollars on ineffective therapies in a so-far fruitless effort to come up with an effective treatment for dementia and Alzheimer’s.

Now, in the latest of a steady drumbeat of research that suggests diet, exercise and socializing remain patients’ best hope, a study published recently in the Proceedings of the National Academy of Sciences shows that vitamins B6 and B12 combined with folic acid slowed atrophy of grey matter in brain areas affected by Alzheimer’s disease.

Alzheimer’s disease and dementia mostly affect older people. As people live longer, the number afflicted by the conditions is growing. Delaying dementia with an inexpensive vitamin regimen may help stem the surge in cases, which the World Health Organization predicted would more than triple from 36 million worldwide in 2010 to 115 million in 2050, as well as the cost, estimated at $604 billion in 2010 by Alzheimer’s Disease International.

Vitamin Market

Vitamin makers and retailers such as Pfizer’s consumer health-care unit and GNC Holdings Inc. (GNC) in the U.S. and Reckitt Benckiser Group Plc and Holland & Barrett Holding Ltd. in Europe stand to benefit. The Nutrition Business Journal estimates the global market for vitamins, minerals and supplements was $30 billion in 2012 and forecasts sales will grow 3.6 percent by 2017.

In the PNAS study, researchers tracked 156 people ages 70 and older who had mild memory loss and high levels of a protein previously linked to dementia. Among people with elevated homocysteine, the study found that the amount of grey matter declined 5.2 percent in those taking a placebo, compared with 0.6 percent in those who took the vitamin cocktail. The supplements cost about 30 cents a day in pharmacies and health-food stores.

First Look

The U.S. Food and Drug Administration hasn’t cleared new drugs for memory loss conditions in a decade. Approved medicines such as Eisai Co.’s Aricept ease symptoms without slowing or curing dementia. A joint U.S.-European Union task force in 2011 found that all disease-modifying treatments for Alzheimer’s in the previous decade failed late-stage trials despite enormous financial and scientific efforts.

Since then at least four more experimental treatments have failed, but research continues. 

Shrinking Brains

Older people’s brains shrink about 0.5 percent a year from the age of 60, and faster in people with vitamin B12 deficiency, mild cognitive impairment or Alzheimer’s disease. If that pace can be significantly slowed before full-blown Alzheimer’s develops, it may delay the disease’s progression so that older people can enjoy better lives until they die from another cause.

“If you delay the onset by five years, you can halve the number of people dying from it,” says Jess Smith, a research communications officer at the Alzheimer’s Society, a U.K. charity.

The Oxford group studied people in the Oxford, England, area who had mild cognitive impairment, also known as MCI, or some memory loss. One in six people over 70 have MCI and about half of those develop dementia within five years. Alzheimer’s accounts for 50 percent to 80 percent of all dementias, according to the Alzheimer’s Association.

Vitamin Cocktail

Study volunteers were given either a placebo or 0.5 milligrams of vitamin B12, 20 milligrams of vitamin B6 and 0.8 milligrams of folic acid. Their brains were scanned using magnetic-resonance imaging and blood levels of the protein homocysteine were measured at the start of the trial and two years later. The MRI scans compared how much grey matter was lost in brain regions most affected by Alzheimer’s disease.  Results very clearly showed that subjects taking the vitamin cocktail had significantly lower loss of grey matter.

Brain Atrophy

The research reinforces previous findings that supplements slowed brain atrophy and cognitive decline in the group.

Smith and his colleagues at Oxford reported in 2010 that the atrophy rate in patients’ whole brains was reduced about 30 percent in those taking the vitamins and 53 percent in those on the vitamins who also had elevated homocysteine. They published study results in 2012 of memory tests that found people on the vitamins who had high homocysteine were 69 percent likelier to correctly remember a list of 12 words.

Vitamin B12 is found in liver, fish and milk and folic acid in fruit and vegetables. Deficiency of folate and B vitamins is already linked to dementia. Researchers such as Smith are studying whether less-than-optimal levels of B vitamins and folic acid contribute to its development.

When you’re buying a suit, the most important factor is fit. This cannot be stressed enough. Fit, fit, fit. Consider it a mantra. You can drop $1,000 or more on a suit, but if the shoulder pads extend well beyond your shoulders or you can comfortably fit a baby between you and your jacket, the suit has lost almost all its value. Conversely, an inexpensive suit can look like a $1,000 suit if it’s perfectly tailored. The price point and designer label of your suit should be academic. It is all about the fit, color and fabric.


Here are the five suits in descending order of importance:


The Wool Navy Suit



If you only have one suit in your wardrobe, make it a single-breasted navy wool suit. Solid navy implies professionalism and respectfulness—respect for yourself and your audience, whether your audience is small (you’re meeting with a colleague) or large (you’re giving the State of the Union address). Plus, have you ever noticed navy looks good with literally everyone’s complexion?

Rule: Don’t worry about labels so much as quality of material. Suits are graded by fineness of wool, which is reflected in the “super” number. You can often find it on the tag, but if you’re not sure, ask. Look for a super number in the low to mid 100s range (finer and lighter than the courser 80s and 90s but not as expensive, thin, and easily snag as the upper 100s and higher).


The Cotton Khaki Suit



The suit for spring and summer is cotton khaki in a sandy color; not too dark, but not too pale, which shows every spot. Wear it to work with a crisp white button down and a colorful pocket square; dress it down on the weekend by removing your jacket and wearing a plaid shirt you with the sleeves rolled up.

Rule: Never wear a cotton khaki suit during the fall or winter, and consider your office’s dress code before wearing it to work; it’s considered less formal than wool.


The Wool Charcoal Suit



Like navy, charcoal is supremely versatile, looks excellent with brown, black, and burgundy shoes and belts, and can be dressed up or down depending on the occasion.

Rule: If it’s a two-button suit, only button the top button. Dry clean it as infrequently as possible (the chemicals they use to dry clean shortens the life of your suit), and hang it up after every wearing on a proper wooden hanger. Please do not use wire hangers.


The Patterned Suit



A pattern allows you to show a bit more personality without going overboard, and unlike solid mid-grey (which isn’t as slimming as charcoal, black or navy), actually helps distract from bulges.

Rule: If the pattern of your suit is very visible from afar (glen plaid, for example), stick with solid shirts and ties; if it looks solid from afar, treat it like a solid. Give it a squeeze in the store. Quality fabric should spring back without creasing.


The Black Suit



Instead of buying a tuxedo as the fifth suit, you’ll get more for your money with a suit in classic black. The benefit of a black suit versus a tux is that it can be worn not only to black tie weddings and cocktail parties, but to sober gatherings, too.

Rule: At a funeral, go with a white shirt and black tie. Save colors, patterns, and flashy accessories for festive occasions. And keep in mind: navy and grey are more appropriate for work or interviews.


Final words on tailoring



There’s a pretty good chance that, even if you’re the same size as the models, you left the store with imperfectly fitted suits. Maybe the sleeves fall past your knuckles or there are about three breaks of fabric at the bottom of your pants. That’s fine for suits you’ve just purchased. But don’t wear them until they've been tailored. Tailor does not mean the “very nice guy” at your lovely dry cleaner who hems all your pants. The only person with whom you should entrust your beautiful suits is a professional tailor who does this for a living, and even then, not just any tailor. Do your homework and find the best one. Ask the best dressed guy you know where he gets his clothing tailored and start there. Once you locate him, all you have to do is show up and stand there while he starts pinning, transforming your suits into masterpieces.

President Obama recently announced a proposal for the BRAIN initiative, a research project with lofty goals to map every neuron in the human brain. The idea is comparable to what researchers did with the Human Genome Project, with a similar aim of creating a massive science resource. What happens in between, however, proves far more complex. Whereas the human genome is made up of 20,000 to 25,000 genes, the human brain contains almost 100 billion neurons making trillions of connections. The research will be long and grueling, but the end product could be monumental for understanding brain disease and advancing technology. However, the president’s proposal is hardly novel: a group of Europeans announced a similar plan last year and researchers have been hunting down solutions for brain diseases like Alzheimer's and autism for years. As the world races for answers, it’s important to note where brain research currently stands. So the question is: what do we know? Where are we starting? The years ahead may reveal a world-is-round-not-flat epiphany in the brain realm, but for now, below is a presentation of the basics and how we currently understand the brain.
